I really like the way you have designed this experiment. You might be interested to know that there is a whole discipline called survey methodology that deals with the design of questionnaires. This picture shows several survey methodologists from the 1950's conferring. I think that the trick to designing a good survey instrument is to choose questions that will be very easy for the lazy person to answer, but then provide space for elaboration by more forthcoming respondents. You also have to hook them with an interesting subject line (as someone mentioned in class), and be very humble and nice.
